The CSC requires all government employees and officials to submit two copies of their accomplished PDS to their agency HRMO not later than April 30, 2017. The agency HRMO shall then submit one copy of the employees PDS to the CSC Field Office concerned.

Make sure to write your name in capital letters and use the format Last Name, First Name, Middle Name. If you have dual citizenship, you will need to specify the countries of your citizenship and attach a copy of your Certificate of Retention/Re-acquisition of Philippine Citizenship.



  • Family Background. In this section, you will provide the names and details of your spouse (if married), children (if any), parents, and siblings (if any). You will also indicate their occupation, employer/business name, business address, and contact numbers. If you are married to a foreigner, you will need to attach a copy of your marriage contract.



  • Educational Background. In this section, you will provide the names and addresses of the schools you have attended from elementary to college (or post-graduate studies if any). You will also indicate the period of attendance, the highest level or units earned, the year graduated (or expected year of graduation), and the course or degree obtained. If you have any honors or awards received during your studies, you will need to list them as well. If you have any civil service eligibility or professional license/certification, you will need to indicate them in this section as well.



  • Civil Service Eligibility. In this section, you will provide the details of your civil service eligibility or professional license/certification that are relevant to the position you are applying for or holding. You will indicate the type of eligibility/license/certification, the rating (if applicable), the date of examination/issuance/conferment/expiration (if applicable), and the place of examination/issuance/conferment (if applicable). You will also attach a copy of your certificate of eligibility/license/certification.



Work Experience. In this section, you will provide the details of your work experience in both government and private sectors.
